1. Assumption of Risk
I understand that participating in Jiu-Jitsu, martial arts, and fitness activities involve physical contact and inherent risks, including injury, permanent disability, or death. I voluntarily assume all such risks.
2. Medical Fitness
I certify that I am physically able to participate and have no medical condition that would prevent safe participation, and/or I have received physician clearance.
3. Release of Liability
I release First Degree L.L.C., its instructors, staff, and affiliates from any claims arising from injury, loss, or damage during training, except for gross negligence or willful misconduct.
4. Emergency Authorization
I authorize academy staff to obtain medical treatment for me if needed and accept full responsibility for costs.
5. Media Release
I consent to photos/videos taken during classes or events being used by the academy for marketing or educational purposes.
6. Agreement
I have read, understand, and agree to this waiver. By submitting electronically, I acknowledge that my electronic signature is legally binding.
